Media Orphaned Instead of Attached to Post

Description

After upgrading from v2.7 to v2.8.4, image-to-post attachment seems to work at random. As I wrote in the forum (no replies after 2 days), when I click "Insert Into Post" the image appears, but doesn't get attached to the post unless I do it a second time.
