Errors in the post editor, in the kitchen sink, and for adding links

Getting a bug on the editor in 3.2 (and 3.2.1) - vanilla install, no plugins running, default admin theme.
Using Chrome 13 (not dev)

Two issues

1. I get both sets of icons on the toolbar - both the visual and the HTML - see a screenshot here - ​http://www.uploadscreenshot.com/image/388997/1456831.

2. When entering links in visual I get glitchy visual errors - doubled up X button, weird white opacity strip - ​http://www.uploadscreenshot.com/image/388998/859402.

When I click Update, I get taken straight back to the post list, link creation having failed. The only way to make links is to do it in code.

Exactly the same issues in Firefox 4, and Firefox 5.

Resolved, due to dodgy front end theme screwing up the admin display
