Steps to reproduce: 

1. Install trunk / 3.4
2. Login to wp-admin, navigate to Posts > Categories or Posts > Tags (edit-tags.php)
3. Note the inconsistent alignment between the heading (Add New Category) and the form elements + headings (e.g.. Name etc)
4. UHOH
