Gallery option - clean code

The code used by the standard gallery (shortcodes) leaves something to be desired, although I can understand that you would want people who just turn it on to get something that looks decent.
There are two (at least) options that it makes sense to implement site-wide, rather than on a page by page, or gallery by gallery basis:
- Using a 'clean code' version, which does not add an inline styling block, and uses a list rather than a group of definition lists.
- Whether to link to the 'attachment page', or straight to the full size image. The latter would be very useful for people who'd like to use a lightbox style presentation, rather than using sub-pages.
