Give Menu Items a default "draft" state when added to the menu

Description

Per discussion with Jane and Ryan in IRC, menu items should have a draft state when added to the menu, which means they are not visible publicly.

Then when either the individual menu item is saved or the entire menu is saved with draft menu item attached, that menu item becomes "published."

There is no UI for changing draft state, so publishing is a one-way action.
