Add New Category has broken user feedback

Description

The Add New Category form in the Categories admin page appears to be completely dead. Seemingly, nothing happens - no slug autofill, no response to clicking the Add New Category button.

However, it seems to be working nearly fine. Refresh the Categories screen and the new category is in the list.

I've verified this on two sites that I upgraded to 3.4.

Do you see any JavaScript errors on the page? (Your browser should have a JavaScript console which will be useful for this, e.g. F12 in Chrome).

Have you tried disabling all plugins and retrying?

Hi, no javascript errors (Chrome). In Wordpress 3.4.1 now, the form still shows no indication of success, but the category list does visibly re-fresh. Haven't tried disabling other plugins yet.

Adding a new category works exactly the same for me in 3.2, 3.3, and 3.4.

I don't think we ever had slug autofill in that form.

There's indeed no success message, but the new category should appear at the top of the list.

This sounds like expected behaviour.
