Contribute link on Credits page

Should "Get involved in WordPress." on the credits page link to the new Make page? It's linking to this page in the Codex at present: ​http://codex.wordpress.org/Contributing_to_WordPress.

Replace link to Codex with link to new Make page. Updated patch: created from root directory, instead of from within folder.

Since make.wordpress.org is only available in English, it's probably not necessary to keep the URL as a translatable string.

However, the Codex page is translated into several languages, so I guess some translation teams might still want to link to a page with instructions on contributing in their own language.

Remove redundant translator comments, add comments for placeholders instead.
