Description
If the first post does not have a featured image set, the post title is hidden by the fixed nav bar when 'Skip to Content' is used.

I believe this is what the OP is referring to. When the "Skip to content" link is used, it takes you to the opening #content element. In this case, the fixed header overlaps the top part of div#content that has the title and category.
This regardless if there's a featured image set, except if there is one, the top of it gets cut off a bit.
