Description
Hi there,
Hope you well?
When I Add Media and insert Title, Description, Caption and Alt text and then Insert in post or page, the <img...> renders with alt text but NO caption, title or description (where is descrition used, just for WP admin knowledge?)
Though, if I edit an alread-inserted image, the caption is blank and if added and update, it adds the caption shortcode.
Hope this little bug is resolved soon.

I have deactivated ALL plugins (i only have Akismet, Atom Twitter Feeds, NextGEN Gallery by Photocrati, Responsive Photo Gallery, Theme Check, Ultimate Tag Cloud Widget, WordPress SEO and WP Parallax Content Slider) Still ONLY applies the src and alt ... no caption, title. Though only applies the caption
A few colluegues are also experiencing the same ... Caption doesn't add on Add Media ... I am on Windows 7 using Chrome 36.0

This is happening to me in WP 4.0 with no plugins activated. The caption does not appear when inserting an image from the Add Media button. Using Chrome 37.0.2062.94, Mac 10.9.4. I have to re-add the caption to the inserted image in order to preview it.
UPDATE: Try switching your theme to "Twenty Fourteen" --the captions will stick in the admin editor as expected. Must be an issue with your theme, as it seems to be mine.

It's been a month, and this seems to be theme-related - I just tried a couple dozen scenarios and got the caption shortcode every time.
If this still happens on the default theme for someone, please reopen.
Thanks for the report Yatish. I've tested this and I'm unable to reproduce the problem. A caption is inserted along with the image, as expected.
Do you have any plugins active on your site which may be interfering? Can you try disabling them one by one?
Which browser and OS are you using?
