camaran, what version of WP did you activate Multisite on? (as of 3.5, if you activated Multisite, you didn't use ms-files.php, so I want to be sure you were originally pre-3.5 and we hunt down the bug the right way).

camaran, can you get back to us with an answer please? Was Multisite activated on your site prior to or after WordPress version 3.5?

ms-files.php hasn't changed in a long while, but something connected to it may well have.

Can you test your site with all plugins deactivated and the default theme enabled?

  • Resolution set to worksforme

I've just tested a multisite upgrade from 3.4 to 4.1, with existing media in place and with newly uploaded media (which are all served through ms-files.php) and I've not seen any issues.

I'm going to close this this as worksforme as we don't have any other details to go on.
