generating new sub domain in a WordPress multisite installation

I have a big issue with a wordpress mu site. and before you say, start again with a fresh installation, I cannot because the site is a number of years old.

SiteX is a multisite installation. When I create a new sub domain site say: i.e. its says Site added. Visit Dashboard or Edit Site. But the link to the dashboard points to route domain i.e. and the visit dashboard points to the dashboard.
The link under Site Address (URL) is which when typed into the browser says Error establishing a database connection.

WordPress expert wpfixit gave up after a couple of hours of trying to solve the problem.

Does the issue still happen with all plugins disabled and a default theme (Twenty Seventeen) activated?
