Remove underlines from links that shouldn't have it

Description

Now that text-decoration: underline is set in global.css for all body links, the underlines need to be removed from all the places they shouldn't appear. Changes per design mockups.

See #6117 for double underlines in the Dashboard. There are also some underlines in various buttons that I'm not sure should be there.

Write a New Post, Write a New Page, and Change Theme in Dashboard.

Change and Remove in Widgets admin.

Do we want links in the theme and plugin lists in the Theme editor and Plugin editor?

TinyMCE: add internal command and shortcut (Alt+Shift+X) for toggling <code>. Define a button that can be added to any toolbar as wp_code. See #6113
