Reloading the DB upgrade confirmation screen causes upgrade routine to go from b2 to 2.8, messing up password hashes

Description
If you reload the DB upgrade confirmation screen (step=1), db_version will be db_upgraded, and WP will transport you back to 1955 and make your mother fall in love with you. Also, it messes up your password hashes.

(In [11040]) Drop the dual-use of db_version. Now it just stores the version. Post-upgrade hook triggered by separate binary db_upgraded option. fixes #9618
