admin network error

I have some problem in the network admin,

First:
the user bar on top don't show

Second:
Appear this error in footer section
Fatal error: Cannot redeclare add_cssclass() (previously declared in /home/a5084417/public_html/wp-admin/includes/menu.php:127) in /home/a5084417/public_html/wp-admin/includes/menu.php on line 130

The second one was probably fixed in [16027].
