Media Library: "Uploaded To" information incorrect

Description

If I upload an image through the Media Library, it does not show "Uploaded To" information in grid mode, and in list mode it shows "(Unattached)". After I use it in a post it suddenly shows "Uploaded To: New Post". That doesn't make any sense.

I'll have to jog my memory for the particulars, that's actually exactly how it was designed to work in 3.5.

This was an intentional change in [22843], see #22085.

Whether or not attachments should be attached to a post if they're added unattached, I don't know. But if this is the best behaviour (?), then the wording is wrong. I never uploaded that image to that post.

I suppose the wording is weird, but assigning unattached media on inserting into a post has been there a long time, pre-3.5. Not sure where to go with this.

I can't imagine a scenario where [gallery] with no attrs continues to work if we change this behavior.
