Description
Just for information, and I hope it’s a “one-off” instance, when updating one of my sites today, every image was removed and replaced with “placeholder” text. This applied to both background images and normal images on every page of the site.
I am using Oxygen6, and the previous WordPress version was the most recent up-to-date version. Hosting is with Bluehost (who are not the best).

I have rectified this by opening the image location in each case and, as the placeholder text related to the correct image, in situ in the media file, uploaded it again. It was clearly still linked to the page via the placeholder, but the image itself had been removed. It applied to every image on the site, both background images and normal uploaded images.

This type of thing would happen if, for example, the URl to the image was changed somehow or the image was no longer in the same location.
To narrow possible causes, is the issue still present if you disable all plugins and switch to a default theme?
