PDF previews generated when uploaded through post/page, but won't insert thumbnail into content

Description
When uploading a PDF through the "Add Media" button while editing a post or page, a preview of the file is generated just fine (I can see it in the Media Library).
However, when I select an attachment size and click "Insert to page", only a hyperlink with the media title is inserted. No matter what attachment size I select, I cannot get the preview image to insert into my page.
